Our office participates with numerous insurance plans.  Please call for more information.  We deliver the finest, most appropriate care at the most reasonable cost to our patients; therefore, payment is due at the time service is rendered unless other arrangements have been made in advance.  If you have questions regarding your account please contact us at 315-997-3044.  Many times, a simple telephone call can clarify anything that may seem unclear. 

However, please remember you are fully responsible for all fees charged by Mattydale Oral & Facial Surgery regardless of your insurance coverage.  Any remaining balance after your insurance has paid is your responsibility.  Your prompt remittance is appreciated.  We can make arrangements for a monthly payment plan, but this must be implemented prior to the actual procedure. 

We accept the following forms of payment:

• Cash

• Major credit cards (Visa, MasterCard, American Express, Discover)

• Debit cards

• Flexible Spending Accounts (FSA) / Health Savings Accounts (HSA)

• Third-party financing (e.g., Cherry, Proceed Financing – if applicable)

We work with many insurance providers and will gladly file claims on your behalf.

Please note:

• You are responsible for any portion not covered by your insurance, including co-pays, deductibles, and non-covered services.

• An estimated out-of-pocket cost will be provided before your procedure based on the information available from your insurance company.

• Insurance benefits are not guaranteed; final responsibility for payment lies with the patient.

• For consultations: at the time of service.

• For surgeries: full payment of your portion is due prior to the procedure, unless other arrangements have been made or your insurance covers your services 100%.

• We understand that life can become busy at the most inconvenient times, but we request at least 24–48 hours' notice for cancellations or rescheduling.

• Late cancellations or no-shows may be subject to a fee.
